Main causes
- Ovulation problems — most commonly PCOS; also thyroid disorders, raised prolactin, premature ovarian insufficiency, excessive exercise, low body weight, severe stress.
- Tubal disease — blocked or damaged fallopian tubes. Pelvic inflammatory disease (including after Chlamydia or gonorrhoea), tuberculosis of the pelvis (particularly relevant in India), previous surgery, endometriosis.
- Endometriosis — can cause ovarian and tubal damage.
- Fibroids and polyps — depending on size and location.

- Age-related decline in egg quantity/quality — the strongest single factor after about 35.
