Endometriosis is a condition where tissue similar to the lining of the uterus grows outside it — on ovaries, fallopian tubes, bowel, bladder, or the lining of the pelvis. This tissue responds to the menstrual cycle, so it bleeds and inflames each month, causing pain, scarring and sometimes infertility. It is common — around 1 in 10 women of reproductive age — and often diagnosed only after years of being dismissed.
Symptoms
- Severe, worsening period pain — often the first clue.
- Pain during sex.
- Chronic pelvic pain, between periods.
- Heavy or irregular periods.
- Pain with bowel movements or passing urine, especially during periods.
- Tiredness, low mood.
- Difficulty becoming pregnant — sometimes the first feature in women with silent disease.
Why it is often missed in India
"Period pain is normal" is a cultural assumption that delays diagnosis by 7-10 years on average in many countries, and longer in some Indian communities. Women who are dismissed by one Health Expert should go to another. Endometriosis is under-diagnosed, not uncommon.
Diagnosis
- A detailed history (pattern of pain, family history).
- Examination and pelvic ultrasound.
- MRI in selected cases.
- Laparoscopy with biopsy is the gold standard, but not always required to start treatment.
- A trial of hormonal suppression is often reasonable when imaging is suggestive.
Treatment
- Pain management — NSAIDs at the right time.
- Hormonal therapy — combined pills, progesterone, hormonal IUD, GnRH analogues. Aim: suppress the cycle, let lesions shrink.
- Surgery (laparoscopy) — removes or ablates the endometrial tissue; helps pain and fertility.
- Fertility treatment — many women with endometriosis conceive with help (IVF where needed).
- Pelvic physiotherapy, mental-health support — under-used but valuable.
- Lifestyle — regular exercise, anti-inflammatory diet patterns, managing stress, adequate sleep support treatment but don't replace it.
There is no cure yet, but endometriosis is genuinely manageable. A specialist with experience in endometriosis (gynaecologist, sometimes an endometriosis-focused clinic) can substantially change quality of life — don't settle for less.
